All appointments are scheduled in
advance. When you call to schedule an appointment, the
receptionist will inquire about the patient's medial symptoms and other
pertinent information to ensure that you receive the necessary length of
time for you visit. If you are a new patient, please arrive fifteen
minutes early for your initial appointment.
If you are unable to keep your
appointment, please give us a 24 hour advance notice of cancellation to
allows us to schedule another patient who needs medical attention.
Patients who do not show up for a scheduled appointment may be charged a
service fee.
We realize that your time is valuable and we make every effort to
see patients at their scheduled time. Occasionally, however, our office falls
behind schedule. We ask for your patience and understanding.
Please
call before arriving without an appointment. We ask that you understand
that without an appointment you may not be seen.

First Time
Patients
If you are a first-time patient, please plan on arriving early in order to
complete a patient registration form and please bring the following items to the
appointment:
- Your insurance card
- Physician referral forms if required by insurance
- A list of current prescriptions and/or over-the-counter medications you
are taking, including dose and frequency
- Medical history
- Any recent test results
